Alvesta municipality reports itself after father's attack on daughters
A father in Alvesta attacked his two daughters the day after the end of the school year. He shot the girls, who sustained serious and life-threatening injuries respectively.
The following day, the father was found dead near the house.
The girls' mother had expressed concern for the children to social services earlier in the same week. She had warned of the father's aggressive behaviour and said she was afraid the situation would worsen.
Alvesta municipality has now submitted a lex Sarah report to the IVO, the Inspectorate for Healthcare and Social Care. The municipality is thus reporting itself.
The report states that the circumstances suggest there may have been shortcomings in how the case was handled. The municipality does not rule out that what occurred constitutes a serious misdemeanour.
An external actor will conduct the investigation.
The relevant administrative director in the municipality does not wish to comment on the case.
